Perhaps transition is change from one state to another, where both are
known, as in Newark to Chicago by plane. Just one example, of course.
Transformation may be like leaping into the void. No net.
Transition may emphasize the change of venue. Transformation, on the
other hand, may be more about the person or organization doing the change.
On the other hand, maybe not.
