Based on the assumptions in your posting, you might ask these questions?

Does a learning organization assume that an individual can address a
performance deficiency caused by a process?

Does a learning organization think that there is a single cause for each
performance deficiency?

Does a learning organization suppose that every performance deficiency is
detectable and treatable at each and every moment of its existance?
