The above could be based on the assumption that:

* Linear & Sequencial Thinking on e.g. work flow or
material / product flow or info / data flow

* and using the Linear & Sequencial Logic to detect
error of prior process before proceeding with
next process

However if System Thinking is applied, error need not be
result of Linear or Sequencial Flow but can be
result of Over Time and Space, even from other remote
process that seems insignificant.
