Some alternate views on Change and some reading suggestions:

Teams and change - use diagonal slices to achieve functional integration

Change can occur at any level, and is especially powerful if generated by
personal and individual transformation

The Burke-Litwin model of organizational performance and change offers
some interesting ideas on the concept of transformational change in
organizations, and identifies the specific transformational change
variables as being: the external environment, leadership, mission and
strategy, organization culture, and individual and organizational
performance

Suggested selected reading:

Managing Change, William Bridges

Integrated Strategic Change, Christopher Worley, et al

Diagnosis for Organizational Change: Methods and Models, Ann Howard
(includes info on the model mentioned above...)
