Well . . .
Documentation is not the solution, IMHO. It is the problem.
Who will gather the facts, look up the document, gleen the
knowledge contained therein, and then take the knowledgeable
actions so indicated. By that time the problem (facts) has/have
changed.
Let the computer note the fact changes, run the knowledge, and emit
action recommendations.
