On the question of storing knowledge, I offer a well-indexed database of
corrective and preventive actions. Such a system is required by formal
quality systems standards such as ISO 9000 and Mil-Q-9858. If the proper
tools and methods are deployed across an organization and corrective
action procedures are analytic methods (as opposed to accusatory
nonsensicalities), we have the beginnings of a learning repository. Such
efforts should begin locally in large organizations (ie: at the team
level) to avoid the potential of human and information system overload.

A well constructed preventive and corrective action system uses structured
problem solving to get to the root cause of a problem, various
brain-storming techniques and their correlate quantificational tools to
manage breakthrough thinking, etc. Since all information required by a
formal quality system needs to be indexed, collated, and easily
retrievable, the problem is trivial (insert some ironic humor sign here).
Although viewed as somewhat stifling, restrictive, expensive, and
sometimes even superfluous, the basis of most quality standards is a
common sense approach to the identification of control points and feedback
mechanisms in an organization. By looking at the models given by ISO,
DoD, ANSI, and others, we can see their potential in an enlightened
organization for learning and learning history.
