This is useful - in fact, it is the essence of involvement and quality
improvement to have an immediate checkpoint to analyze what went right and
wrong and why.

The essence of an emergency causing empowered behaviour is also
experienced in other command & control businesses. It is almost as if the
"boss" will tolerate empowerment when things are bad enough!
