Both Knowledge for Action and Overcoming Organizational Defenses are good
books. Knowledge for Action is probably more for the individual, the
other for facilitators of the process.

Some of his HBR articles are the best way to start:

Skilled Incompetence (Sept-Oct '86)
Double-loop Learning in Organizations (Sept-Oct '77)
Interpersonal Barriers to Decision Making (March-April '66)
