I'm not a trainer, but the following book is directly applicable to
solving the problems that occur when the training process appears to
have failed. This book is a little gem that I came across many years ago.

I believe it is published by the American Management Association, and
it is a short paperback book titled "Analyzing Performance Problems".
Its subtitle is "or... You Really Oughta Wanna". It lays out
a flowchart by which to diagnose those performance bottlenecks that
seem to show up as "lacks motivation", "doesn't have high standards",
"doesn't take direction" and so forth, and connects them to flaws in the
environment of the task or in the communication about the task.

It's short, it's to the point, and it's absolutely lucidly written,
no jargon. It is great for helping to unsnarl practical problems (such
as lack of resources or lack of information) that have gotten obscured
by emotional reactions.
