Its been my experience that accountability is often used as
the politically correct synonym for punishment, which always
brings to mind a quote from a great systems thinker:

Where there's injustice, I've always beleived in fighting. The
question is, do you fight to change things or do you fight to
punish? For myself, I've found we're all such sinners we should
leave punishment to God. And if we really want to change things,
there are better ways of doing it than derailing trains or
slashing someone with a sword. -- Gandhi

Most managerial efforts toward accountability equate to
slashing someone with a sword. Gandhi also had a lot to say
about responsibility.
