IBM has had a system like this for at least five years. Unfortunately,
last time I looked it was mainframe-based and was accessible through
PROFS/Officevision. You might want to check with IBM Learning Systems
(sorry don't have a number!) to see if they have/will be taking advantage
of distributed computing technologies and graphical user interfaces or
their new groupware products (Lotus Notes).

You will need to think through how you want "this to occur within the
philosophy behind Learning Organisations" so you can be more explicit in
describing what you want from your information technology solution. As an
implementer of IT solutions and familiar with the learning organization
idea, I would have a hard time satisfying your desires based on the
requirement as stated. We'd need to spend a lot time developing the
requirements and prototyping the solution.
