request for quality information

I don't know if this is the proper forum and if not then I apologize
for taking the space, but I have been asked to complete a quality
related questionaire. I have some specific ideas relative to the
government service organization I'm affilliated with, however I am
more interested in others experiences and/or opinions on the subject.
I appreciate all the assistance I can get. Thank you in advance.

IN A SERVICE ORGANIZATION HOW WOULD YOU:

QUANTIFY THE COST OF ERRORS

MEASURE CUSTOMER SATISFACTION/PRODUCT ACCEPTANCE

MEASURE PRODUCTIVITY

ESTABLISH CUSTOMER REQUIREMENTS
