Parish Census

Below is the Immaculate Parish Census form. Please open it, print it out and fill it out in pen or pencil. The form is not yet set up for being processed on line.

To finish filling out the form please tell us if there is anyone at home who, because of sickness or old age, is unable to get to Mass and who would like a priest or lay eucharistic minister to bring him or her Holy Communion. If there is someone, please write this person's name on the back of the form with a note regarding communion.

Further, we wish to visit parishioners of the Immaculate Conception Parish who may be in a nursing home or a hospital. If there are any parishioners in your family who is presently in a nursing home or hospital, please list that person's name and the name of the nursing home or hospital on the back of the form.
